When to refer
Refer by diagnosis, symptom pattern or clinical question. EVSLA can review the referral when the final treatment pathway is not yet clear.

The clinical question matters as much as the diagnosis. These details help the team route the referral and prepare for the patient's visit.
Full name, date of birth, phone number and preferred language when relevant.
Describe the symptom pattern, working diagnosis and what you would like evaluated.
Note rest pain, tissue loss, rapid progression, infection concern or another time-sensitive finding.
Include vascular studies, cross-sectional imaging, laboratory results and prior procedure reports when available.
Include the current medication list and other information relevant to procedural planning.
Attach the referral order and authorization when required by the patient's plan.

Questions from referring offices
Include the patient's demographics, the reason for referral, symptom pattern, urgency, relevant examination findings, current medications, allergies, insurance information and any available imaging or procedure reports. A clearly stated clinical question helps EVSLA route the case appropriately.
Common pathways include arterial disease, chronic limb-threatening ischemia, non-healing wounds with a vascular concern, venous insufficiency, varicose veins, suspected venous obstruction, aneurysm evaluation and selected embolization consultations for fibroids, pelvic venous disease, BPH and varicocele.

Call EVSLA directly for a time-sensitive but non-emergency concern so the referral can be triaged. Suspected acute limb ischemia, stroke, pulmonary embolism or another medical emergency requires immediate emergency evaluation and should not be submitted through an online form.
The referral workflow is designed to return relevant consultation findings and follow-up information to the referring office. Confirm the preferred secure delivery method and contact details when sending the referral.
Requirements vary by plan and service. Include insurance details, the referral order and any authorization already obtained. EVSLA should verify benefits and advise the patient or referring office when additional authorization is needed.
Patients may contact EVSLA to request an evaluation, but insurance plans can have different referral or authorization rules. Referring clinicians should use this page's secure pathway when transferring clinical information.
Use only EVSLA's approved secure portal, secure fax process or other authorized transmission method. Do not send protected health information through a standard web form or ordinary email unless EVSLA has specifically approved that secure workflow.
